Pre-Procedure Requirements

Patient Preparation

  • Complete coagulation profile (PT, PTT, INR)
  • CBC with platelets

Equipment Preparation

  • Core needle biopsy set (14-18 gauge)
  • Fine needle aspiration supplies (22-25 gauge)
  • Sterile field setup
  • Local anesthetic (1-2% lidocaine)
  • Specimen containers with fixative
  • Imaging guidance equipment as needed

Procedure Steps

1. Initial Assessment

  • Verify patient identity and biopsy site
  • Review relevant imaging
  • Mark optimal biopsy approach
  • Position patient appropriately

2. Sterile Technique

  • Perform surgical hand scrub
  • Don sterile PPE
  • Prepare site with chlorhexidine/betadine
  • Apply sterile drapes

3. Biopsy Execution

  1. Administer local anesthetic
  2. Make small incision if needed
  3. Insert biopsy needle under guidance
  4. Obtain adequate tissue samples (minimum 3-5 cores)
  5. Apply pressure for hemostasis

4. Specimen Handling

  • Label specimens immediately
  • Document number and size of cores
  • Place in appropriate fixative
  • Submit rush specimens to pathology

Post-Procedure Protocol

Immediate Care

  • Monitor vital signs q15min x1hr
  • Assess biopsy site
  • Provide post-procedure instructions
